**We will request your most recent pay stubs and bank statements as we move your case forward.
After your free Case Review / Initial Strategy Session Please provide the following:
- STEP 1 Review and Sign your Professional Service Agreement - Look for an email with the link to your agreement.
- STEP 2 Complete the Bankruptcy Questionnaire - Look for an email with the link to your questionnaire. Call with questions any time along the way.
- STEP 3 Complete the First online Credit Counseling Course - Visit www.moneysharp.org and use Attorney Code: PZLAW
- STEP 4 Provide Supporting Documents - Below are list of required documents needed to draft and prepare your bankruptcy petition and documents.
- Original Driver’s License and Original Social Security Card
- Household Income Documents: Provide all the below documents that apply to you.
- Pay Stubs: 6 months of pay stubs leading up to the bankruptcy filing date
- If you are self-employed: 6 months of Monthly Profit and Loss Statements (Call and we can send you a sample P&L form)
- Award Letters: Social Security Income or Disability, Pension, Food Stamps, etc.
- Contribution Letter: Roommate contribution to the household monthly bills
- Financial Account Statements : 6 months of statements for the following accounts:
- Bank statements- Statements for every bank account you have
- App statements – Any apps where you have deposited funds (Venmo, PayPal, CashApp, ACORN, Etc.)
- Retirement Accounts (401K, IRA, Etc.) – provide your most recent statement
- Tax Returns: State and Federal Tax Returns - Provide the last 2 years of returns for chapter 7 and the last 4 years of returns for chapter 13.
- All tax refunds received during the bankruptcy must be turned over to the Trustee.
- Vehicle Information
- Pictures of all vehicles (front and side, back and other side, any damage, or scratches). Just email these pictures from your phone to the address below.
- Vehicle Registration Certificate – Provide a copy or a picture of your CURRENT registration certificate.
- If Applicable provide the following:
- Decree of Divorce, if you have been divorced within the last 8 years
- Summons/Complaints, Judgments and/or garnishments - If you are being sued or garnished please provide what documents you have.
- Mortgage Statement- Provide the most recent statement
- Auto Loan Statement or Auto Lease Agreements - Provide the most recent statement
- Apartment Rental Agreement
